Please read the following instructions thoroughly and let us know if you have any questions.
You will receive an email from teamcaredonotreply. It will contain a link to our portal and instruct you to reset a password. The username is the first name of the individual receiving services and the year of birth. (Example: Alex1984) You will select your own password. Please access this link in the next 48 hours or it will expire.
Once the portal is established, you will see the forms that must be completed. Should you need to leave the forms and go back later to complete them, you will be unable to access the portal through the link a second time. You will navigate to teamcarebh.com, scroll down the page and click on the red box labeled Established Patients, and then click on Client Portal. You will sign in with the username and password you previously set up to return to the portal and complete the forms. Please save your username and password as you may need the portal for future communication.
