STEP 1:
Interested in services? Click here to start a request/referral for services.
STEP 2:
Accessing the Client Portal:
***You will receive an email 24-48 hours after submitting your “Request for Services Form”. ***
- Check daily the email that you submitted on the “Request for Services Form” for an email from “teamcaredonotreply”. This email contains your one-time link to the client portal. This link will expire in 48 hours.
- Click on the link to take you to the portal.
- The “Username” is the first name of the individual receiving services, followed by their birth year. For example: John1984
- You will then follow the prompts to create your own password.
- Once you have successfully created your new login, please complete each of the intake forms. Please save your Login information in a safe place for future communication.
***Please note that if you are unable to complete all the documents on the first attempt, you will not be able to access the forms again through the same portal. Instead, please navigate to TEAMCare’s website www.TeamCareBH.com and click on the “New Patients” tab. Scroll to the bottom of the page and click on the “Click Here to be Redirected to the Client Portal to Complete Your New Patient Paperwork button.” Sign in with your Username and password.*** - When you have completed the forms, please call TEAMCare’s office to schedule an “intake” appointment.
Please call us if you have any questions or have not received your login email from “teamcaredonotreply” 48 hours after submitting your “Request for Services Form”.
